Maternity safety intelligence

See the whole picture, before harm occurs.

OBSee brings a mother's scattered clinical data into one continuous view, surfacing where deterioration goes unrecognised and escalation is delayed, so teams can act in time.

Built clinician-first and safety-case ready, in response to the Ockenden Review and Baroness Amos's National Maternity Investigation (2026).

About OBSee

Maternity safety intelligence

The Ockenden Review and Baroness Amos's National Maternity Investigation confirmed what every maternity unit fears: babies and mothers are still being harmed by failures that were visible in the data all along. The warning signs are usually there (in the notes, the bloods, the CTG, the triage call), but scattered across separate systems that don't talk to each other, so no one sees the whole picture in time.

OBSee brings that fragmented data into one continuous view. It's an awareness engine that assembles a mother's trajectory across existing systems and flags where deterioration went unrecognised and escalation was delayed, surfacing concern for a clinician to act on. It never overrides the clinician; it makes sure nothing gets missed.

We start with a retrospective audit tool that shows each trust exactly where escalation is failing, using their own data (including where triage calls diverge from the record and where escalation timeliness differs across ethnic groups), then turn those patterns into targeted, blameless learning for the team. The audit is designed to generate evidence that supports trust reporting against the Maternity Incentive Scheme (MIS), to strengthen the governance a trust already performs, rather than adding a new burden.

See the founding team below. We're looking for a lead trust partner and early supporters to prove it where it matters.

The problem

Maternity harm is rarely a knowledge gap. It's a recognition and escalation gap.

The Ockenden Review and Baroness Amos's National Investigation found the same failures recurring: systems that don't talk to each other, deterioration unseen across fragmented records, escalation delayed, and women's concerns, especially from Black, Asian and minority ethnic families, dismissed.

162
potentially avoidable deaths (156 babies, 6 mothers)
500+
further instances of potentially avoidable harm
2,500+
family cases reviewed, the largest in NHS history
50%

"The data needed to see the deterioration usually exists. It just isn't assembled, surfaced, or heard in time."

The approach

One continuous thread of awareness

OBSee is an integration layer that augments your existing maternity systems. It never replaces them. It assembles the picture and shows the diverging trajectory; the clinical decision stays with the clinician.

Assembles fragmented data

Prior birth history, antenatal records, bloods, observations and CTG, brought into a single trajectory across systems like BadgerNet, the EPR and the lab.

Surfaces delayed escalation

Flags where a mother's trajectory diverged and escalation lagged: the recurring pattern behind avoidable harm.

Carries the patient's voice

A mother's own concern enters the record directly and cannot be silently overridden: the principle behind Martha's Rule, built into the workflow.

Surfaces concern, never certifies safety

Designed so it cannot be used to license inaction. Silence is never read as safety, and grounding is in physiology, not historical behaviour.

Surfaces inequity for review

Disaggregates escalation timeliness across groups so disparity is visible and can be investigated: a signal for human-led review, never an autonomous judgement. Aligned with the national Perinatal Equity and Anti-Discrimination Programme.

Supports the response after harm

Compiles the case timeline for post-birth reflection meetings: the same trajectory data, put in service of the mother's own account, not just service-level audit.

Regulatory alignment

Evidence for the Maternity Incentive Scheme

The Maternity Incentive Scheme (MIS) Year 8 streamlines trust reporting to six safety actions. OBSee is designed to generate evidence that can support trust reporting and governance against three of them.

Safety Action C

Learning from reviews

Escalation-delay cases and time-to-review gaps, assembled from CTG, triage-call and observation timelines: the raw pattern a thematic report is written from.

Safety Action D

Service-user voice & equity

Escalation timeliness and delay rates disaggregated by recorded ethnicity, surfaced for review, not diagnosis.

Safety Action F

Board oversight

Case-level findings aggregated into an overall delay rate and trend, ready for the six-monthly board safety review.

The roadmap

Each stage earns the right to the next

We ship the retrospective audit first, then build toward live awareness, on validated logic and a tuned signal. Each phase is scoped by regulatory risk, lowest first.

Wedge

Audit

Retrospective pattern analysis of a trust's own historical data. Assembles fragmented records (BadgerNet, EPR, LIMS, CTG) into a single timeline per case, and flags where a deteriorating trajectory wasn't escalated in time, including where triage calls don't match documentation and where escalation timelines vary across ethnic groups. Also designed to generate evidence that can support trust reporting against the Maternity Incentive Scheme (MIS).

Provides the case-level evidence every later stage builds on.

Response

Post-birth review

Compiles the case timeline for post-birth reflection meetings: the same trajectory-assembly technology, put in service of the mother's own account rather than service-level audit alone. Directly supports women after a complex birth, answering Baroness Amos's recommendation to improve the response when harm occurs.

Extends the audit's trajectory engine to patient-facing use.

03
Culture

Learning loop

Turns anonymised real cases into blameless, team-based learning modules. Surfaces what got in the way of timely escalation, not who, generating targeted training the team can act on directly from the audit's own findings, rather than a static report nobody reads.

Turns audit findings into culture change without needing new data sources.

Non-device: regulatory position to be confirmed before deployment
04
Equity

Triage voice

A text-only scribe for triage calls that flags where what a woman said doesn't match what was documented or escalated: implementing the spirit of Martha's Rule in software. One of the clearest available signals that a concern wasn't taken seriously, a pattern Baroness Amos names directly in her findings.

Captures the patient-voice data the live flagging stage will need.

05
Live

Active flagging

Real-time deterioration and escalation-delay flagging, built last and only once the underlying logic has been validated against retrospective data from every earlier stage. Requires a full clinical safety case and device pathway before any live clinical use. This is the long-term vision, not where we start.

The awareness engine running as care happens, not just after the fact.
